GLP-1 Medications and Muscle Loss

GLP-1 medications, such as Ozempic, Wegovy, Mounjaro, and Zepbound have changed the way obesity and diabetes care are treated. These medications work by mimicking a hormone that helps regulate blood sugar, slow digestion, and reduce appetite. As a result, many individuals experience significant weight loss and improvements in metabolic health. However, rapid weight loss can also lead to loss of lean body mass.

Muscle loss is not unique to GLP-1 medications and can occur with any form of weight loss, including dieting, bariatric surgery, or calorie restriction. However, because GLP-1 medications significantly reduce appetite and calorie intake, individuals may unintentionally consume fewer nutrients needed to maintain muscle tissue. Preserving muscle is important for metabolism, strength, physical function, and long-term health outcomes.

Adequate protein intake plays a key role in protecting muscle during weight loss. Protein provides the amino acids needed for muscle repair, enzyme production, and many other essential body functions. When calorie intake is low, the body may begin breaking down muscle tissue for energy. Consuming sufficient protein can help reduce this process and support maintenance of lean body mass.

Carbohydrates are also an important part of preserving muscle due to what is known as the protein-sparing effect. Carbohydrates are the body’s primary source of energy. When carbohydrate intake is too low, the body may break down protein from muscle tissue to produce energy. Eating adequate carbohydrates allows protein to be used for its intended roles in muscle maintenance and metabolic processes rather than being used as fuel.

A helpful analogy is to think of the body as a house. The structural framework of a house is made from strong plywood, which represents protein in the body. Carbohydrates act like firewood used to fuel a fireplace. When heating a house, we would not burn the structural plywood that holds the house together. Instead, we burn firewood. In the same way, carbohydrates provide energy and help “spare” protein so the body can maintain muscle structure and function.

In addition to balanced nutrition, resistance or strength training plays a critical role in preserving muscle during weight loss. Activities such as weight training, resistance bands, or bodyweight exercises help stimulate muscle growth and maintain lean body mass. Strength training also provides health benefits beyond muscle maintenance. Research notes that resistance exercise can help slow age-related bone loss and even build bone density, which reduces the risk of fractures and osteoporosis. Strength training can also improve balance, stability, and physical function, helping individuals remain active and independent as they age.

Another important consideration with GLP-1 use is the shift in the eating experience that some individuals report. A reduced sense of hunger or enjoyment of food can feel unfamiliar or even concerning, but this is often a result of changes in the brain's reward pathways. In practice, this experience can be supported by normalizing the adjustment, encouraging individuals to explore other sources of pleasure and connection, and reframing meals as opportunities for connection and nourishment rather than focusing on the food itself. Emphasis on mindful eating and prioritizing the quality of food can also help individuals maintain a positive and sustainable relationship with eating.
